Dr. Ofer Azoulay

Ofer Azoulay, M.D., FACS, is a board-certified Otolaryngologist–Head and Neck Surgeon with advanced fellowship training in microvascular reconstruction. He specializes in the comprehensive surgical management of head and neck cancer, with particular expertise in complex oncologic resections and functional
reconstruction using free tissue transfer. His clinical focus is delivering precise, patient-centered care while restoring form and function after cancer treatment.

Dr. Azoulay is recognized for building and leading high-performing,
multidisciplinary teams that manage the full spectrum of head and neck pathology—from advanced tumors to reconstructive challenges. He has performed hundreds of free flap reconstructions, including jaw, tongue, pharyngeal, and skull base defects, with outcomes grounded in evidence and surgical excellence.

In addition to his oncologic practice, Dr. Azoulay leads specialized programs in facial nerve reanimation and sleep surgery. He co-founded and established a translational research lab exploring nerve repair strategies. His work integrates surgical innovation with patient quality-of-life outcomes. His sleep surgery team has treated dozens of patients using Inspire® Hypoglossal Nerve Stimulation and
transoral robotic techniques, offering alternatives to CPAP for selected individuals with obstructive sleep apnea.

Prior to joining Hackensack Meridian Health, Dr. Azoulay served as Chief of Head and Neck Surgery at SUNY Downstate. He remains active in research and medical education, with a focus on surgical training, cancer survivorship, and nerve regeneration.

Dr. Azoulay sees patients with head and neck tumors, complex reconstructive needs, facial paralysis, sleep-related breathing disorders, and other ENT diseases.
