Course Overview
The Level IV Complications & Revision Fellowship is ORCAA’s most advanced and exclusive surgical program — designed for experienced implant surgeons seeking mastery in the management of complex full-arch complications.
This is not a traditional educational course.
It is a high-level surgical immersion, focused on real-world decision-making, advanced revision strategies, and the clinical judgment required to navigate challenging cases.
Participants will work directly alongside expert faculty in a live surgical environment, gaining exposure to complex complications encountered in both conventional All-on-X and advanced zygomatic and pterygoid treatments.
This program is reserved for clinicians already operating at a high level — and looking to refine the most difficult and nuanced aspects of implant surgery.
- Format: 4-Day Live Surgical Immersion
- Prerequisite: Completion of Level III or equivalent experience (Must be proficient in zygomatic and pterygoid implant placement)

What You’ll Learn / Do
Clinical Exposure May Include
- Management of failed All-on-X cases
- Revision of zygomatic and pterygoid implant treatments
- Managing biologic complications (infection, soft tissue breakdown, bone loss)
- Surgical decision-making in complex, non-ideal scenarios
- Re-treatment planning and execution in compromised cases
- Occlusal and biomechanical considerations in revision cases
- Intraoperative problem-solving and adaptation
- Advanced soft tissue and surgical management techniques
Hands-On Surgical Experience
- Extremely limited enrollment: Only 2 doctors per program
- High level of surgical involvement and case exposure
- Direct, real-time feedback and guidance from faculty
- Exposure to a variety of complication scenarios across full-arch treatments
Program Format
- 4-Day Intensive Surgical Immersion
- Hosted multiple times per year (approximately every 3–4 months)
- Rotating locations across partner surgical centers in Brazil
